Below is a form to fill out for the item(s) you are donating or have procured for donation to our event. 

*Please make item name/description specific to the item(s) itself for inventory and labeling purposes. If you have a basket of goodies and have a suggestion for a name, please list that in the item description field, in parenthesis with a note 'Suggested Basket Name'. 

*If you are donating a dessert for our dessert auction, please fill out the appropriate name and description in the appropriate fields and be sure to include the name of the bakery/company donating.   The same info request as above applies, surrounding filling out info if you are an APPLE parent recording a procured dessert or a donor.
